Dear Parents/Guardians:
Happy last full week of summer! School starts next week on 8/18/21, YAY!
As a reminder, Illinois School Code and High Mount policy require all NEW students entering Pre-K, all Kindergartners, 6th graders and incoming transfer students have medical records up-to-date per State mandates. We have received some forms, but many of our Kindergartners and 6th graders are still missing paperwork. Students' records transferring from another IL district have been requested by the main office; if we do not receive paperwork from your child's previous school, it is your responsibility to obtain and turn in. Please disregard this email your kiddo's medical paperwork is current.
Items of most concern are physical exams and proof of immunizations up-to-date. Attached is a list of local clinics for those that may need assistance. Vision exam forms may be turned in through 10/15/21. Dental Exam forms may be turned in through 5/15/22, but are encouraged ASAP. Dental Safari will be coming in November and those kiddos that are seen through this service will receive the required form at the time of evaluation.
Students not in compliance with State requirements will be excluded until required documentation is turned in. This means if your child comes to school on the first day of school without medical records up-to-date, they will not be able to attend class and must be picked up immediately. This includes those students with incomplete medical records who were remote last school year.

Medications: ALL medications including prescription or over-the-counter (OTC) will not be given without a physician’s order. If your student needs to take medication at school (including OTC), we must have a HMS Medication Authorization Form completed and signed by medical provider and this includes cough drops, ointments, acetaminophen (Tylenol), ibuprofen (Motrin). MEDICATION MUST BE TRANSPORTED BY PARENT/GUARDIAN.
Asthma Kiddos: All students with Asthma must have an Asthma Action Plan and Medication Authorization completed by physician, signed by parent, renewed annually at start of each school year. If your child needs nebulizer treatments, parents must supply tubing, mask or mouthpiece with albuterol solution; HMS will provide nebulizer.
Allergy/Food Allergy Kiddos/EpiPens: It is parent/guardian’s responsibility to verify that their student’s allergies are accurate and up-to-date yearly with the school. All students with food allergies must have an Allergy Action Plan and HMS Medication Authorization Form for EpiPen and/or antihistamine or any other intervention indicated by Action Plan.
Dietary Restrictions: As required by State of Illinois through school lunch program, if your child has a dietary restriction, we must have a note from their physician with the specific restriction noted, renewed yearly.
Seizure History: Students with seizure history must have a Seizure Action Plan and intervention medication if advised in Seizure Action Plan. Seizure Action Plan should be completed by their physician and signed by parent on file, renewed annually.
Diabetic or other Health Concerns: Students with Diabetes Mellitus must have Care Plan on file and provide HMS with all supplies necessary to accommodate daily needs, including Glucagon.
I will be available in my office at Open House on 8/16/21 to turn in forms, medications, etc.

All try outs will take place at Mel Price Park.
Students must have an updated sports physical on file before trying out.
Students must bring a completed permission form to try outs (attached).

Parents:
On July 5th, online registration will be available for any returning students. Once you register your child you will need to email me your proof of residency (recent utility bill, lease, etc.) Your enrollment will not be approved until your proof of residency is received. To register you will need to login to your Teacherease account using the email you provided to the school office.
If you would rather register in person please call the school office to make an appointment. All new families and incoming Kindergarteners must register in person.

Dear HMS Families,
We made it! It is hard to believe that we are nearing the end of the 2020-2021 school year. In some respects it has been the most memorable year of our lives, and in others it has been one of the most challenging. Everyone at HMS, teachers and students alike, have worked diligently to embrace the sweeping changes in which we were presented. Though it was not always pleasant, we persevered and forged ahead. Together, we made it through a year of ups and downs, unexpected firsts, and living in a world of “what if?” The 2020-2021 school year will go down in the history books as one in which we were all stretched outside of our comfort zone. Yet, we have all learned and grown through this season. The quote “it takes a village” has never felt more true. Most sincerely, I thank you enough for your continued flexibility and support.
What is next?
Good luck to our 2021 eighth grade class. Keep reading, pursue your curiosity and dreams, and please come back to visit!
